URGENT: Required Pre-Class Checklist – Class Starts Wednesday , November 12th
Hello all,
You must complete the items below before attending class this upcoming Wednesday, November 12th. If your name appears on the list for any of the required items, you must complete it immediately. Failure to complete these steps by the due date will result in removal from the program.
1. Submission of ID Documents (Driver's License/SS Card)
Due: ASAP
Action: If you have not submitted a copy of your Driver's License, California State ID, or Social Security Card to your student portal, please do so immediately. This documentation is required for the office to submit your application to the CDPH.
Consequence: Failure to submit will cause a delay in receiving your CNA License.

2. Live Scan Processing
Due: Friday, 11/7
Action:
You should have received a copy of your Live Scan form with your enrollment paperwork. Print this form out.
Take the printed form to the Sheriff's Department (refer to the "Live Scans" announcement for the address).
If you cannot print the form, please call or email the office immediately, and we will print it for you.
Once processed, bring a copy of the completed Live Scan back to the MJ Nursing Office.

3. Canvas Account & Orientation Quiz
Due: Monday, 11/10
Action:
On your announcements page, use the link provided in "Canvas & Zoom Link/ Orientation Quiz" to create your account.
Once logged in, watch the orientation video and take the Orientation Quiz.
You must achieve a perfect score (100%) on the quiz. Retake it as many times as necessary to score 100%.

4. Tuition Payment
Due: Wednesday, 11/12- BEFORE CLASS BEGINS AT 8am
Action: You have already been sent an invoice for your tuition. Please ensure that your payment is made by this deadline to secure your place. If you have issues with payment, contact the office immediately.

Action: To begin class this Wednesday, you must have the Nursing Assistant Textbook.
